this is very vague question with reply that can go on for many pages in larger companies you usually have dedicated role for that, could be process improvement manager/change manager/transformation manager or something like that then you have process owner who is responsible for existing process - documentation/implementation/communication/trainings/quality control etc.. this is person who is (preferably) expert on that specific topic
